Where to Fill a Cash-Pay GLP-1: Direct vs. Retail vs. Mail-Order
Price verification — October 26, 2026
This article includes an older pricing snapshot. Older amounts, coupons and annual totals below are historical illustrations, not current checkout quotes.
- Wegovy (semaglutide): Wegovy tablet: $149/30 tablets at 1.5 mg, $199 at 4 mg (a temporary $149 4 mg offer may apply), and $299 at 9 mg or 25 mg. Wegovy injection: eligible new patients may pay $199 per 28-day fill for the first two fills at 0.25 or 0.5 mg through Dec. 31, 2026, then generally $349 for standard pen doses. Eligibility and dosage restrictions apply. Official source
- Zepbound (tirzepatide): LillyDirect self-pay: 2.5 mg starts at $299/28 days, 5 mg at $399, and 7.5 mg and above at $449 only with qualifying 45-day refill timing. Higher-dose prices can be higher when those conditions are not met. Supplies may cost extra. Official source
- Clinical program fees: Ivím lists a $74.99/month membership with medication billed separately. Sesame lists care from $59/month on a 12-month commitment; month-to-month costs more, and medication is separate. These are care fees, not all-inclusive drug prices. Official source
- Sesame annual-commitment terms: Official source.
These are manufacturer self-pay or listed care-fee examples, not one all-inclusive price or guaranteed prescription. Insurers, clinical visits, pharmacy choice, dosage, supplies, refill conditions and geography can change the amount due.
For brand-name GLP-1s paid in cash, the manufacturer's self-pay program sets your price, not the pharmacy. As of October 2026, the same prices Novo Nordisk and Eli Lilly charge through their own pharmacies are listed at major retail chains when the manufacturer offer is applied. The costly mistake is paying through a generic discount card instead, which can cost three to four times more for the same supply.
Key takeaways
- Novo Nordisk lists Wegovy pens at $349 a month ($199 for a new patient's first two fills) and the Wegovy pill at $149 to $299, through December 31, 2026.
- Lilly lists Zepbound self-pay at $299, $399 and $449 by dose for the KwikPen or vials, with the $449 KwikPen price tied to refilling within 45 days and the current card ending December 31, 2026.
- GoodRx lists those manufacturer prices at chains including Kroger, Walgreens, Walmart, Costco and CVS inside Target.
- Discount-card prices are far higher. SingleCare listed a Foundayo 0.8 mg coupon at $601 or more versus the $149 manufacturer price.
- Amazon Pharmacy does not publish GLP-1 cash prices on its public homepage, so compare its checkout total with the manufacturer price.
The three channels, and why price is usually the same
Manufacturer-direct pharmacies are NovoCare Pharmacy for Novo Nordisk drugs and LillyDirect Pharmacy for Lilly drugs. Novo says its self-pay prices are available at NovoCare Pharmacy "without a savings offer or insurance" (NovoCare, Wegovy offer). Lilly's Zepbound savings page links to LillyDirect Pharmacy and to a separate enrollment site for other pharmacies (Lilly, Zepbound savings).
Retail pharmacies can charge the same price when you use the manufacturer's self-pay offer. Novo says to enroll online or text SAVE to 83757, then present the offer to your pharmacist (NovoCare). GoodRx's drug pages show the manufacturer-sponsored prices across ten chains (GoodRx Wegovy; GoodRx Zepbound).
Mail-order pharmacies such as Amazon Pharmacy may apply manufacturer coupons too. Amazon says eligible manufacturer coupons are applied automatically at checkout. Its public homepage, however, lists no GLP-1 cash prices; prices appear only after you search for a medication (Amazon Pharmacy).

Manufacturer-direct: what you get and what to watch
Novo Nordisk (Wegovy, Ozempic)
Novo's Wegovy self-pay price is $349 a month for every pen dose from 0.25 mg to 2.4 mg, after a $199 introductory price for a new patient's first two fills at the 0.25 mg and 0.5 mg doses. Wegovy HD 7.2 mg is $399. The pill is $149 for 1.5 mg, $199 for 4 mg and $299 for 9 mg and 25 mg. A month means one 4-pen box or one 30-tablet bottle. Novo says the $199 introductory price runs through December 31, 2026, and that pricing "will be updated after December 31, 2026" (NovoCare).
For Ozempic, Novo lists $199 for a new patient's first two fills, then $349 for 0.25 mg to 1 mg and $499 for 2 mg (NovoCare Ozempic). Self-pay purchases don't count toward your insurance deductible. Novo says people with government insurance may still pay cash outside their coverage.
Eli Lilly (Zepbound, Foundayo)
Lilly's Zepbound KwikPen self-pay prices are $299 for 2.5 mg and $399 for 5 mg. The 7.5 mg through 15 mg doses cost $449 with a purchase offer, compared with regular prices of $499 for 7.5 mg and $699 for 10 to 15 mg. The $449 price applies only if you refill within 45 days of your previous fill. The card allows up to 11 fills per calendar year and ends December 31, 2026. Lilly adds that the $299 starting price covers the KwikPen or vials, "not Zepbound single-dose Pen" (Lilly Zepbound savings).
Lilly sells Zepbound vials at the same $299, $399 and $449 tiers, a price cut it made in December 2025 (BioSpace). For Foundayo, approved by the FDA on April 1, 2026, Lilly announced self-pay pricing "starting at $149 per month for the lowest dose" (Lilly, Foundayo approval). That announcement did not list prices for higher doses, so they are left out here.
Retail pharmacies: same price, different trade-offs
GoodRx's October 2026 pages list Zepbound at $299 and Foundayo at $149 at each of ten chains: Kroger, Meijer, Walgreens, Walmart, Giant Eagle, Costco, CVS inside Target, Sam's Club, Discount Drug Mart and Walgreens Specialty Pharmacy. Both are labeled as Lilly-sponsored self-pay offers (GoodRx Zepbound; GoodRx Foundayo). The Zepbound page doesn't break the $299 out by dose, so expect Lilly's higher-dose prices to apply at higher doses.
The Wegovy page lists pens at $199 for 0.25 mg and 0.5 mg, $349 for 1 mg through 2.4 mg and $399 for 7.2 mg, plus Novo's pill prices (GoodRx Wegovy). For Ozempic, the listed pen prices are $199, $349 and $499 by strength (GoodRx Ozempic).
The advantages of retail are practical: same-day pickup when the drug is in stock, a pharmacist you can talk to in person, and keeping all your prescriptions in one place. The risk is the claim being run the wrong way at the counter. Ask the pharmacist to process it under the manufacturer's self-pay offer, not a discount card or your insurance.
Why discount cards are the wrong tool here
Generic discount cards are built around list prices, and brand GLP-1 list prices are high. On October 19, 2026, SingleCare showed a Wegovy 0.5 mg pen prescription (listed as quantity 1) at $1,086 to $1,384 with its coupon, depending on pharmacy (SingleCare Wegovy). It listed 30 Foundayo 0.8 mg tablets from about $601 with its coupon (SingleCare Foundayo). Compare that with the $349 and $149 manufacturer prices. Discount-card pricing tools are useful for many generics, but for these drugs the manufacturer offer wins by a wide margin.
Mail-order pharmacies
Amazon Pharmacy is the best-known mail-order option for cash-pay customers. Its public homepage describes Prime discounts, the RxPass subscription for eligible medications and automatic application of eligible manufacturer coupons, but shows no GLP-1 prices (Amazon Pharmacy). Because it doesn't publish a price, compare the checkout total against the manufacturer price before you pay.
Costco appears on GoodRx's pharmacy lists at the manufacturer prices above.
Where TrumpRx fits
TrumpRx.gov lists the Wegovy pill at $149, Wegovy pen at $199, Ozempic at $199 and Zepbound at $299 a month, with no dates or dose breakdown shown on the page (TrumpRx.gov). We cover TrumpRx in detail separately. For pharmacy choice, the point is the same: these are manufacturer cash prices, not pharmacy prices.
How to choose
- Lowest price with the least friction: Go manufacturer-direct, or bring the manufacturer self-pay offer to a retail pharmacy. The listed prices are the same.
- Need it today: Use a retail chain listed for the offer, and call ahead to confirm your dose is in stock.
- Prefer home delivery: Check the manufacturer-direct option or a mail-order pharmacy, and confirm the manufacturer price applies before you pay.
- Have commercial insurance that covers the drug: A copay card may cost less than cash. GoodRx shows Novo and Lilly copay offers as low as $25 for eligible commercially insured patients (GoodRx Ozempic; GoodRx Foundayo).
- Plan for January 2027: Both Novo's and Lilly's current self-pay terms reference December 31, 2026.
The verdict
Choose your pharmacy for convenience, not price. As of October 2026, the manufacturer's self-pay offer sets the cost of Wegovy, Ozempic, Zepbound and Foundayo, and that price is available from the manufacturer's own pharmacy and, per GoodRx, at major chains including Walgreens, Walmart, Kroger and Costco. Use a retail pharmacy if you want same-day pickup and in-person advice. Use the manufacturer-direct channel if you want the offer applied with the fewest steps. Whatever you choose, make sure the claim runs under the manufacturer offer and not a discount card, which can cost several times more. Confirm Zepbound refill timing and recheck prices after December 31, 2026.

Frequently asked questions
Is Wegovy cheaper at NovoCare Pharmacy or a retail pharmacy?
The listed self-pay price is the same. Novo says its self-pay prices apply at NovoCare Pharmacy, and you can bring the savings offer to your own pharmacy. As of October 2026, pens are $349 a month after a $199 introductory price for a new patient's first two fills.
Can I get Zepbound for $299 at Walmart or Costco?
GoodRx lists Zepbound at $299 under Lilly's self-pay offer at ten chains, including Walmart and Costco. Lilly's $299 price is for the 2.5 mg KwikPen or vials. Higher doses are listed at $399 and $449.
Should I use a discount card like SingleCare for a brand GLP-1?
Usually not. SingleCare listed Foundayo 0.8 mg from about $601 with its coupon and, in October 2026, a Wegovy 0.5 mg pen prescription at $1,086 to $1,384, far above the $149 and $349 manufacturer prices.
Does Amazon Pharmacy publish GLP-1 cash prices?
Not on its public homepage. Amazon says eligible manufacturer coupons are applied at checkout, so compare your checkout total with the manufacturer's listed price.
Will these cash prices change in 2027?
Possibly. Novo says Wegovy pricing will be updated after December 31, 2026, and Lilly's Zepbound self-pay card ends December 31, 2026.
