Dose-Increase Economics: Who Charges More as You Titrate (and Who Doesn't)
GLP-1 medication works by gradually increasing your dose over weeks to months. At some providers, every dose increase comes with a price increase too. Here's who stays flat and who doesn't.
Titration is how GLP-1 medication works. You start at a low dose and increase gradually — typically every 4 weeks — until you reach a therapeutic or maintenance dose. For semaglutide, that's a journey from 0.25mg to 2.4mg. For tirzepatide, from 2.5mg to 15mg. The process takes 16–20 weeks for most patients.
At some providers, your monthly cost stays the same throughout this entire journey. At others, each dose step brings a higher bill. The difference between flat and tiered pricing can be hundreds of dollars over a 6–12 month treatment period — and it's rarely disclosed on the landing page.
The Dose-Pricing Table
| Provider | Pricing Model | Starting Dose Price | Maintenance Dose Price | Increase |
|---|---|---|---|---|
| Gala | Flat across all doses | $179/mo (tirz) | $179/mo | $0 |
| GobyMeds | Flat within bundle tier | $99/mo (sema 12-wk) | $99/mo | $0 in same bundle |
| Wellorithm | Flat | $147/mo (sema tablets) | $147/mo | $0 |
| Found | Flat | $189/4wk | $189/4wk | $0 |
| Embody | Intro → ongoing jump (not dose-based) | $79 intro | ~$299/mo | Jump at month 2, not per dose |
| MadeMed | Format-based, not dose-based | $179/mo (inj sema) | $179/mo | $0 within format |
| SHED | Dose-dependent at maintenance | $199/mo (sema inj) | $399/mo (tirz 7.5mg+) | +$100–200 at higher doses |
| Telos Rx | Plan-length + dose dependent | $49 first month | $129–449/mo | Complex — varies by 3 factors |
| MEDVi ⚠️ | Dose-dependent | $179 first (sema inj) | $299–369/mo | +$120–190 at refill doses |
| Yucca | Plan-dependent tiers | $146/mo (6-mo promo) | $275/mo monthly | +$129 when promo ends |
Why This Matters More Than You Think
Most patients don't stay at the starting dose. The whole point of titration is to reach a higher, therapeutic dose where the medication produces meaningful results. That means the price you see advertised — which is almost always the starting-dose price — is not the price you'll be paying when the medication is actually working at full strength.
Consider SHED's tirzepatide pricing: $299/month at starting and intermediate doses, but $399/month once you reach 7.5mg or higher. For many patients, 7.5mg is the maintenance dose where the best results happen. The $100/month jump at exactly the point where the medication becomes most effective is a pricing structure that benefits the provider more than the patient.
Telos Rx is even more complex: your price depends on medication type, dose, and plan length simultaneously. Two patients on the same dose of tirzepatide can pay $129 or $449 depending on whether they chose a 12-month plan or month-to-month. This isn't inherently wrong, but it makes cost prediction nearly impossible without a spreadsheet.
The Flat-Rate Advantage
Providers with flat-across-doses pricing give you cost predictability. You know your month-1 cost is your month-12 cost, regardless of where your prescriber takes your dosing. The standouts:
- Gala — $179/month for tirzepatide at every dose on the yearly plan. This is the single most predictable pricing model for patients who expect to titrate to higher doses.
- GobyMeds — Bundle pricing stays flat within a tier. Your 12-week semaglutide bundle at $99/month is $99/month whether you're on 0.25mg or 2.4mg.
- Wellorithm — $147/month for oral semaglutide tablets at all doses. $249/month for tirzepatide tablets at all doses.
- Found — $189/4 weeks for semaglutide dissolvable tablets, flat across the titration schedule.
The most predictable tirzepatide pricing in the market. $179/month yearly plan, same price at starting dose and highest maintenance dose. No surprise increases.

Before committing to any provider, estimate your total cost through the titration period. Most patients reach maintenance dose within 4–5 months. Here's the formula:
- Months 1–2: Starting dose. Use the provider's lowest advertised price.
- Months 3–4: Intermediate dose. Check if the price changes at this stage.
- Months 5–6: Maintenance dose. This is where dose-dependent pricing hits hardest.
- Add any membership, consultation, or platform fees.
- Subtract any intro discounts (but only from the months they apply).
The resulting number — not the month-1 ad — is the number that matters. For a detailed pricing breakdown by provider, see our first-month pricing audit.
Best for dose-predictable pricing: Gala ($179 flat tirz), GobyMeds ($99/$133 flat bundles), Wellorithm ($147/$249 flat tablets), and Found ($189/4wk flat). Watch for hidden titration costs at: SHED ($100+ jump at maintenance doses), Telos Rx (three-variable pricing), and MEDVi ($120–190 increase at refill doses). Flat pricing isn't always the cheapest month-1 price — but it's almost always the cheapest month-6 price.
Methodology
Dose-pricing relationships verified against provider websites, checkout flows, and Terms of Service documents as of July–September 2026. "Flat" means we could not identify any price change across available dose levels. "Dose-dependent" means the provider's published pricing or checkout flow shows different prices at higher doses.
